Revision rhinoplasty (also called secondary rhinoplasty) is a procedure to correct or improve the result of a previous nose surgery. It addresses both cosmetic dissatisfaction and functional problems that may have resulted from the initial procedure.

Revision rhinoplasty can address:

• An unsatisfactory cosmetic result from a previous rhinoplasty
• A nose that looks pinched, over-reduced, or asymmetric after surgery
• Breathing difficulties that developed after the first procedure

Revision rhinoplasty is more complex than primary rhinoplasty because the surgeon is working with altered anatomy. Cartilage grafts (taken from the septum, ear, or rib) may be needed to rebuild or reinforce the nasal structure. The open technique is almost always used to give the surgeon the best visibility and control.

The procedure is performed under general anaesthetic and typically takes 2-4 hours, longer than primary rhinoplasty due to the additional complexity.

Revision rhinoplasty may be right for you if:

• You are unhappy with the result of a previous rhinoplasty

• At least 12 months have passed since your previous rhinoplasty (to allow full healing)
• You are in good general health and fit for general anaesthetic

• You have realistic expectations, revision surgery can significantly improve but cannot always achieve the ideal result

Before Your Procedure

Your journey starts with a thorough consultation. You will share photographs and details of your previous surgery. Your surgeon will assess your current nasal structure, externally and, if possible, with imaging, to understand what was done before and what can be achieved now.

This assessment may be more detailed than for primary rhinoplasty, as the surgeon needs to understand the altered anatomy. Digital imaging will show you projected outcomes.

During Your Procedure

Revision rhinoplasty is performed under general anaesthetic at a government-licensed hospital. The procedure takes 2-4 hours. Your surgeon uses the open technique for maximum visibility and control. Cartilage grafts may be harvested from the septum, ear, or (in complex cases) a small piece of rib cartilage to rebuild or reinforce the nasal structure.

You stay in hospital for 1 night with nursing care.

After Your Procedure

You will have a nasal splint and internal supports. Your surgeon sees you for follow-up to check healing and remove splints (typically after 7 days). You receive aftercare instructions. If you have opted for the surgery-only package, you will manage your own accommodation during recovery.

Recovery from revision rhinoplasty is similar to primary rhinoplasty, though swelling may take slightly longer to resolve due to the presence of scar tissue from the previous procedure.

Day 1-3: Swelling and bruising around the nose and eyes. Nasal congestion. Keep your head elevated. Pain is managed with prescribed medication.

Day 3-7: Bruising fades. Swelling begins to settle. Avoid blowing your nose.

Day 7-10: External and internal splints are removed. You see the initial shape. Expect significant residual swelling at this stage, revision results reveal themselves more slowly than primary rhinoplasty.

Week 2-4: Most visible bruising has resolved. Swelling continues to improve. Return to work and light activities.

Month 1-6: The shape continues to refine. Scar tissue from the previous surgery means swelling resolves more gradually.

Month 6-18: Your final result becomes apparent. Revision rhinoplasty can take up to 18 months for the final result to fully emerge, particularly in the nasal tip.

Revision rhinoplasty carries a slightly higher risk profile than primary rhinoplasty due to the altered anatomy and scar tissue. However, in the hands of an experienced revision specialist, it is a well-managed procedure.

Common effects (expected and temporary):

• Swelling and bruising, may be slightly more prolonged than primary rhinoplasty
• Nasal congestion, improves as internal swelling settles
• Numbness at the nasal tip, may take longer to resolve
• Mild discomfort, managed with medication

Less common considerations:

• Need for further revision, a small percentage may desire additional refinement
• Cartilage graft complications, donor site discomfort (ear or rib), graft reabsorption
• Structural weakness, the nose has less cartilage available for support
• Asymmetry or irregularities, working with scar tissue adds complexity
• Infection, rare with sterile conditions
